Webinar: Embracing Incentives for Improvement Part I

Overview: Overview: Hospitals are accountable for a variety of financial incentives to measurably improve care quality and efficiency—the “value” of care. In adopting these performance-driven incentives, Medicare and other payers have placed significant weight on improving patient transitions from hospital to post-acute and ambulatory care. This webinar will identify hospital care transitions interventions that will benefit patients and yield financial rewards for hospitals, post-acute, and ambulatory care providers and practitioners.
This is part I of a 2-part program. Part II will focus on clinician and chaplain partnerships to improve advanced illness and end-of-life care. Part II is scheduled for November 14, 2017.
Target Audience This session is intended to benefit hospital and community leaders, physicians, care transitions/readmissions personnel, revenue cycle managers, clinical integration leaders, quality improvement personnel, interested in the Medicare Access and CHIP Reauthorization (MACRA) proposed rule and the new Medicare-Based Incentive Payment System (MIPS) alternative payment models.
