Event Details
Overview:
Home Health Providers seeking success must rewire traditional homecare operations to address the Volume to Value (V2V) transition under Impact Act reforms. By focusing on all elements of the V2V conversion, we can address both the operational and culture changes necessary for success in the Value era. Reviewing the care programming models of Part A Providers outside of Home Health can guide us in the development of a value-based, patient-centered model that addresses all elements of the reforms. Rewiring the HH Operational model allows for us to addresses the traditional homecare method of retro-active care management as we switch to real-time care control. A demo of the operational rewire allows us to identify each value point in the HH model, assuring timeliness and efficiency are monitored in an ongoing manner for outcomes. A summary of the value-based Operational model employs case study demoes to illustrate its’ focus on reform requirements for success in the Value era.
Objectives:
- Address areas of the PPS model that must be addressed for HH reform success
- Identify how traditional HH operations fail under Value-based HH reforms
- Propose w rewired HH Operational model for the Value era
- Demo care production, delivery, and management processes for Value-based HH reforms.

Event Details
The development and advancement of antiretroviral therapy (ART) has transformed HIV/AIDs into a chronic disease, which results in an overall increase in prevalence of HIV/AIDS cases and rise in the number of people living with symptomatic HIV disease. Chronic illnesses have high symptom burdens and increase care needs. The course of illness is characterized by periodic, acute exacerbations that will require an increase of care, and aggressive symptom management during acute and future exacerbation is essential. Prognostication is challenging because these patients are at risk for sudden death but may stabilize for prolonged periods.
