Now more than ever, providers are seeing the impact of social determinants of health (SDOH) on patient outcomes — but it’s something that most health care professionals aren’t trained on how to solve. New CMS health equity requirements will require providers to show how they’re addressing health-related social needs and how they are incorporating that strategy into their work. This requirement represents a huge opportunity to improve patient outcomes at scale but also introduces many open questions on how to do so.
Here’s the good news — you don’t have to figure it out alone. Join us for a free webinar that shares how to address patients’ SDOH needs through social care screenings, personalized resources, closed-loop referrals and robust outcomes tracking.

Event Details
May 14 at 11:00am CST/10:00am MST (90-minute session)
Overview:
The Grief River® model evolved organically out of the popular “waves of grief” metaphor. It makes use of the phases of a river (headwaters, rapids, run, delta, and rejoining the sea) and other water analogies to provide a coherent narrative that places one’s current loss within the context of the entire life journey.
This model allows opportunities for connection and consolation through fostering relationship in nature, as well as offering validation and inspiration by using the expressive arts, religion & spirituality, and philosophy. Hospice and health care clinicians can use this model to provide a compass and additional supplies to companion the bereaved.
Objectives:
- Explain to others the key elements of the Grief River model, which makes use of
nature-inspired metaphors and offers a coherent narrative of loss in the context of the
life journey. - Access additional resources for further study and interventions for use with those who
have experienced a loss. - Be more intentional about the metaphors you use when engaging in helping
relationships
Presenter: Thom Dennis, DMin, LCPC, LMHC, FT, creator of the Grief River® model
With over 30 years of experience supporting individuals and families as they navigate their way through some of life’s most turbulent and troubled waters, Thom brings real-world, practical knowledge applicable to all interdisciplinary team members working in hospice and palliative care. With a professional career immersed in pastoral care and counseling, he has served in various culturally diverse settings, including parish ministry, hospital chaplaincy, and for the past 18 years as a hospice bereavement services coordinator and grief counselor.
Thom is currently on staff at the mental health clinic at Midwest Medical Center in Galena, Illinois, where he also maintains a private psychotherapy practice and offers Grief River ® weekend workshops and retreats. As an artist, poet, and riparian, he strives to integrate into his personal life the wisdom he’s been exposed to as a companion of the bereaved.

May 24 at 12:00pm CST/11:00am MST
Trauma is an emotional response to disturbing or distressing events or circumstances in one’s life, including but not limited to violence, natural disasters, institutionalized oppression, war-related damages, the sudden loss of a family member, or medical injuries. According to the National Council for Behavioral Health, 70% of U.S. adults have experienced a traumatic event in their lifetimes. This accounts for approximately 200 million people in the U.S. living with the effects of trauma.
Despite the prevalence (or incidence) of trauma, many palliative care clinicians have not received Trauma-Informed Care (TIC) education or training. This webinar, presented by Sherika Newman, DO, and Karen Bullock, PhD, fills a critical gap. By the end of this presentation, attendees will be able to:
- Define TIC and Adverse Childhood Experiences (ACEs)
- Understand the importance of TIC in caring for people living with serious illness
- Implement TIC into their practice with new, relevant tools
