The goal of the session would be to demonstrate a successful and ongoing inter-institutional partnership between a community-based hospice organization and a large academic medical center to improve the care of LGBTQ+ patients with serious illness and their families. This includes operationalizing and rolling out the collection of sexual orientation and gender identity (SOGI) in the electronic medical record as well as creating a series of educational interventions aimed at raising awareness, mitigating unconscious bias, and promoting culturally competent and holistic care to a vulnerable patient population.
