The South Dakota Department of Human Services (DHS) would like to remind Assisted Living providers that the claim form for Medicaid payments changed effective Feb. 1.
Assisted Living providers must use the new claim form CMS 1500 for dates of service on or after Feb. 1. For dates of service prior to Feb. 1, claim form UB 04 should be used. Claims submitted on the wrong form will be denied.
