In order to meet South Dakota’s needs for advance care planning (ACP), in 2015 the University of South Dakota’s Department of Nursing assembled an interdisciplinary, collaborative network of health professionals called Advance Care Planning: Quality Conversations. This coalition of health care educators and providers work together to advance strategies to implement ACP across South Dakota.
Recently, the collaborative launched a website, www.advancedcareplanningsd.com, as a way to share information about SPARK (Serving Underserved Populations through Advance Care Planning, Respect and Knowledge). The website provides information about ACP training and workshops and opens the door to other partners who want to join the cause.
