South Dakota Cardiovascular Collaborative releases new resources

The South Dakota Cardiovascular Collaborative (SDCC) released several new resources for facilities working on heart disease and stroke prevention within their communities. The resources include a Team-Based Care Toolkit, Quality Improvement Resource Guide, Cardiac Ready Communities program guide, as well as information on funding opportunities. SDCC would like to highlight South Dakota facilities that are working to improve community cardiac and stroke prevention and treatments. Facilities can share their work in this area by emailing Rachel Sehr.
