The South Dakota Cardiovascular Collaborative (SDCC) released several new resources for facilities working on heart disease and stroke prevention within their communities. The resources include a Team-Based Care Toolkit, Quality Improvement Resource Guide, Cardiac Ready Communities program guide, as well as information on funding opportunities.
