The implementation of Advance Care Planning has the potential to reduce the cost of medical care at the end-of-life, improve patient-centered care and provide collaborative solutions for patients, their caregivers and medical personnel. The South Dakota Coalition for Advance Care Planning: Quality Conversations has come together with the mission of enabling greater access to Advance Care Planning conversations in South Dakota through quality, collaborative healthcare. On April 24th, from 8am-3:30pm at the Ramkota Hotel in Sioux Falls, the coalition will be conducting Respecting Choices First Steps Facilitator Training AT NO COST.
Participants who are interested in becoming a facilitator can register by filling out this form. Participants are required to complete on-line modules prior to the conference and are encouraged to register for the 2nd day of the conference in order to attend the education sessions and keynote speaker on April 25th.
