Healthcare providers play a vital role in preventing and controlling chronic diseases. In South Dakota that includes referring tobacco-using patients to the SD QuitLine​, providing general health related information to patients, ensuring screenings for various chronic diseases take place, using health information technology to ensure overall quality of care, and linking patients to other community health and education resources. The Good and Healthy SD​ website has many new resources for providers on several chronic disease programs that are available for patients. One of those resources is a free rack card that can be ordered for your facility.  To order the rack card, use this link and select the Chronic Disease tab, stock number is CD001.  ​
