The 2019 Chronic Disease Partners and Better Choices, Better Health Meeting will be held October 15th and 16th at the Highland Conference Center in Mitchell, SD. This meeting is a great opportunity to learn about national and local evidence-based programs. The meeting will include presentations from Centers for Disease Control and Prevention on the 6|18 Initiative and the Health Impact in 5 Years (HI-5) Initiative​. Attendees will also hear about the great work occurring​ across the State to impact those suffering from Chronic Disease. Take time to attend and network with previous partners and meet new ones.Â
