The 2021-2022 annual medical facility license renewal process will begin the week of May 17-21, 2021. Emails will be sent out to all licensed providers the week of May 17-21, 2021. The email will include the website link along with the instructions on how to complete the renewal. If you do not receive an email by May 23, 2021 or are unable to access your renewal, please call 605.773.3356. Your online renewal form needs to be completed and electronically submitted to the Department of Health (DOH) on or before June 15, 2021.
Once your renewal has been submitted, please be patient. Several DOH staff will review the renewals to ensure the information they require is included on the application. DOH license approximately 400 medical facilities and appreciate your patience as they review all of the renewals.
