South Dakota Department of Healthcare Facility Inspection Reports Available Online

Effective August 1, all provider survey inspection reports from July 2023 to current final reports are available on the DOH website.

Previously, only nursing home survey inspection reports were available online, while other reports required a request. This change ensures greater transparency and helps families and consumers make informed decisions regarding the care and placement of their loved ones.

Healthcare facility reports now available include:

  • Ambulatory Surgical Center Survey Reports
  • Assisted Living Center Survey Reports
  • Birthing Center Survey Reports
  • Critical Access Hospital Survey Reports
  • End-Stage Renal Dialysis Survey Reports
  • Home Health Agency Survey Reports
  • Hospice Survey Reports
  • Hospital Survey Reports
  • Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICFs/IID) Survey Reports
  • Nursing Facility Survey Reports
  • Outpatient Physical Therapy Survey Reports
  • Psychiatric Residential Treatment Facility Survey Reports
  • Rural Health Clinic Survey Reports
  • Specialty Hospital Survey Reports
As stated on the DOH webpage: Deemed provider surveys are conducted by their respective accrediting organization. The South Dakota Department of Health does not have the authority to post those survey results. Therefore, this survey information is not available on our website.
