A new collaborative hosted by the South Dakota Department of Health called Nexus SD, issued a notice of grant funding for health care, human and social service providers. The purpose of the funding is to encourage providers to sign up for use of the newly developed, integrated technology platform and referral system to help address the social needs of patients and residents. Application submission deadline for this funding is October 18 at 5pm central time. You can learn more about Nexus SD by following them on social media.
