Partnering to advance quality care in South Dakota

Health care providers across South Dakota have a strong partner in quality improvement — the Midwest Quality Innovation Network–Quality Improvement Organization (QIN-QIO).

Led by Telligen and Mountain Pacific, the Midwest QIN-QIO offers one-on-one and group technical assistance, quality improvement webinars and trainings, networking and peer learning opportunities, and data dashboards and customized reports. Because the Midwest QIN-QIO is funded by the Centers for Medicare & Medicaid Services (CMS), organizations that agree to participate in the QIN-QIO program can access these services at no cost.

Midwest QIN-QIO advisors live and work in the region and understand the realities of providing care in South Dakota. They take a collaborative approach, aligning with what facilities are already doing to identify improvement opportunities, develop practical action plans and support sustainable changes that lead to better outcomes for patients, residents and communities.

The Midwest QIN-QIO focuses on six key quality improvement areas:

  • Prevention and chronic disease management – Implementing evidence-based strategies to identify those at risk and working to reduce symptoms, improve self-management and maintain quality of life.
  • Health information technology – Enhancing data exchange and use to strengthen communication across care settings and help patients and families access and use health information.
  • Patient safety – Supporting initiatives to prevent harm, reduce avoidable complications and promote safer care environments.
  • Behavioral health – Addressing conditions such as depression and substance use disorders, which affect patients, residents and caregivers across the continuum of care.
  • Emergency preparedness – Helping organizations build and operationalize system-level plans to respond effectively to emergencies and maintain continuity of care.
  • Care coordination – Improving communication and discharge processes when individuals transition across care settings.

Mountain Pacific has more than 50 years of experience partnering with health care professionals, organizations and communities to drive data-informed decision-making and sustainable quality improvement. Through the Midwest QIN-QIO, that experience is available to providers throughout South Dakota.
