The 2019 Chronic Disease Partners and Better Choices, Better Health Meeting will be held October 15th and 16th at the Highland Conference Center in Mitchell, SD. This meeting is a great opportunity to learn about national and local evidence-based programs. The meeting will include presentations from Centers for Disease Control and Prevention on the 6|18 Initiative and the Health Impact in 5 Years (HI-5) Initiative. Attendees will also hear about the great work occurring across the State to impact those suffering from Chronic Disease. Take time to attend and network with previous partners and meet new ones.
