The state Department of Health, Department of Social Services and Department of Human Services are seeking information that impacts a nursing facility’s decision to admit a nursing home resident.
The information gathered from nursing home providers will be included in the annual review process related to the need for additional nursing facility beds or new nursing facilities.
Facilities are asked to complete the questions contained in the survey and provide comments where applicable. It may be helpful to print a copy of the PDF survey to gather some information prior to accessing the Survey Monkey questions online.
The actual online survey should take less than 5 minutes once the data has been collected. Some questions may not show up on Survey Monkey depending on your response to the previous question. The departments are seeking responses by May 5. Participation is voluntary.
