Typically, the focus on cost-cutting is the common reaction to headwinds . . . 89% of hospitals currently have a cost-reduction plan (but only 6% characterized their programs as effective!) However, growth can have a greater near- and long-term impact on value. Growth can have a compounding effect on value creation and come in a variety of ways: Grow revenue at the same margin (top line); Substitute lower margin for higher margin services (bottom line); Expand volumes in higher margin services (top and bottom line).

On July 30, the Centers for Medicare & Medicaid Services (CMS) released the hospice final rule for fiscal year (FY) 2025. CMS is increasing per diem payments to hospice providers by 2.9%, which is more than the initial proposed increase of 2.6%. The final hospice aggregate cap also increased to $34,465.34 for FY 2025 (up from $33,494.01 from FY 2024). Overview:
This session will ensure that you have full understanding of how to ensure your agency stays off the radar of the Hospice Special Focus Program (SFP). Centers for Medicare & Medicaid Services (CMS) has created a “Special Focus Program” to identify poor performing hospices and help them improve through more frequent surveys and technical assistance. Did you know the SFP actually began January 1, 2024?? Your hospice data TODAY is counting towards the SFP. CMS will also impose enforcement remedies and fines to further incentivize improvement. In this presentation, Annette will discuss the Hospice Special Focus Program and work through the algorithm on what is considered, and how these are weighted- and may land hospices in this “special” program, and steps to ensure you don’t end up in that exclusive community.

Annette is a registered nurse, with a Master’s in Health Care Administration, practicing since 1990. For over a decade she worked with the CMS Home Health and Hospice intermediary, Cahaba GBA and CGS, where she provided review of, and education on home health and hospice documentation. She worked as a specialist in the Medical Review, Appeals, and Provider Education departments, and assisted with cross-MAC quality assurance and training.
Annette then founded Provider Insights, Inc. providing consultative support and provider education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers across the nation with Survey Preparation, ADRs and appeals. Today she continues to marry together her experience in the “real world” and the inside knowledge of Medicare to ensure providers can meet the requirements of CMS, and ensure compliant, efficient operations.

Palliative care access in rural communities can be a challenge, in part, due to workforce challenges. Community health workers can help expand access to other types of care, but what about palliative care? This session will summarize recent research on the perspectives of current palliative care providers and advocates on integrating community health workers into palliative care teams in South Dakota and will suggest opportunities for further discussion.
Presenter: Angela Kroeze Visser, Ph.D., MPH
Angela recently completed her dissertation research on community health workers in palliative care from the University of South Dakota. She also holds an MPH in Community Health Education from the University of Minnesota and has worked for more than 15 years in community public health roles. Her work with communities to identify and address health needs fuels her interest in helping implement programs in ways that are sustainable. In addition, Angela currently serves as the Director of the Kielstra Center for Research and Grants at Dordt University.

This session will provide an overview of workplace safety perspectives in home health care, focusing on the risks of physical assault and non-physical aggression. Participants will learn about the challenges unique to providing services in a home environment and the importance of reporting incidents of violence. The session will also highlight strategies for better preparing staff to handle violent behaviors and the role of leadership in developing a safety culture.
Objectives:
- Explore the Prevalence of Risk: Review the prevalence of physical assault or non-physical aggression that contributes to workplace violence in home care settings.
- Discuss Protection Strategies: Discuss strategies that home care staff and employers can use to protect themselves from workplace violence.

Lisa has served as the Chief of Campus Police and Public Safety with WakeMed Health and Hospitals in Raleigh, North Carolina, Chief of Police and Public Safety with Eastern Virginia Medical School in Norfolk, Virginia and Director of Hospital Police and Transportation at the University of North Carolina in Chapel Hill.
As 2024 Chair of the ASIS Healthcare Community Steering Committee and as a former board president and guidelines council member of IAHSS, Lisa has been fortunate to collaborate with some of the country’s greatest minds in healthcare, security, and policing. She shares her knowledge, strategies, and lessons learned for successfully preventing and mitigating violence in her books: “Preventing Violence in the Emergency Department” and “The Active Shooter Response Toolkit for Healthcare Workers.”

In 2022, CAPC presented a national briefing on the key findings from a national scan to better understand what goes wrong for Black people living with serious illness, and their families. The presenters highlighted that the experiences Black people have are impacted on various levels via the socioecological model. At that time, the presenters shared supporting literature demonstrating that symptom management (both pain and non-pain) provided to Black people living with serious illness is worse than that for White patients.
In this webinar, Brittany Chambers, MPH, and Kimberly Johnson, MD, will share an update on literature since the first briefing, and discuss successful equity-focused interventions, including staff DEI education and training efforts, care navigation, data collection, and community engagement approaches. They will also review evidence-based principles for providing better care for Black people living with serious illness, and their caregivers. Finally, the presenters will offer insights on new policies and payment opportunities that may reduce racial disparities and improve health outcomes.

American Indian and Alaska Natives (AI/AN) experience higher rates of many mental health and substance use conditions than other Americans. The rural nature of most reservations, provider shortages, and lack of culturally relevant care create challenges to delivering appropriate behavioral health services to AI/AN. However, there is a growing body of evidence-based practices to treat AI/AN and more tribes are taking on management of the services for themselves. Some states and tribes are working together to improve the behavioral health care delivered to AI/AN.
This webinar will begin with an overview of the behavioral health delivery system for AI/AN followed by an exploration of two state strategies and offer both state and AI/AN perspectives on each.
