Overview:
The program session will provide an introduction to and demonstration of Nexus SD. Nexus SD is a statewide collaboration of health care, human and social service providers sharing information using an integrated technology platform and referral system to coordinate whole-person care. The Nexus SD vision is to streamline connection between multi-sector providers to address social needs and drive positive outcomes among populations at higher risk and that are underserved.
Objectives:
- Understand what Nexus SD is and how the system works
- Understand the benefits of Nexus SD and how it can help address social needs
- Understand how to join Nexus SD
Presenter: Shauna Batcheller, Care Coordination Program Director, Helpline Center
Shauna has deep knowledge of social determinants of health and passion for implementing innovative and effective programs that improve people’s health and well-being. Much of her time is spent building the relationships needed to integrate our systems of care. Shauna is Certified in Public Health, Nonprofit Management, and is a Certified Resource Specialist. She has her Masters of Social Science in Community Health.
