Overview:
The financial turmoil accelerated by COVID-19 has created a sense of urgency for senior care operators to consider a more sophisticated reimbursement model. The writing is on the wall for fee-for-service and the only reasonable way out is through risk-bearing options. But the only way to succeed at assuming risk is to be able to manage it well. For Hospitals, SNFs, Home Health organizations with limited resources, a one-size-fits-all approach is not effective; instead, we need to hone in on the highest risk residents and ensure we focus resources where they have the best possible outcome for all at-risk. This session will help you look toward the future, with comments on the state and progression of viable strategies taken by other organizations.

3) Explore different risk-based strategies and models other organizations are taking
4) Learn about Frailty as an evidence-based model of risk management
Presenter:
Dr. Steven Buslovich is an Assistant Clinical Professor at University at Buffalo School of Medicine. He is also the co-founder of Patient Pattern, a clinical insight software to help manage clinical outcomes, assess frailty risk, and optimize practitioner decision support for PDPM. He is a practicing geriatrician and medical director of several nursing homes and post-acute care facilities in Western NY. He is an active committee member in the American Geriatrics Society (AGS), Advancing Excellence in Long Term Care Collaborative, Society for PALTC Medicine, currently on the Public Policy and Clinical Practice Guidelines Committee, and has won AMDA’s Shark Tank Innovation Award. Dr. Buslovich is working with CMS and ONC to standardize clinical data elements across all post-acute care settings. Dr. Buslovich is a subject matter expert and has presented nationally on Frailty to audiences from many organizations.
