Dakota at Home: Resources for elderly and disabled

The South Dakota Department of Human Services’ Division of Long Term Services and Supports (LTSS) has a new resource center, Dakota at Home, that provides free information, referral services and assistance to individuals with disabilities, aging individuals and caregivers.

A toll free number, 1-833-663-9673, provides a connection to trained resource specialists who can help identify public and private long-term services and supports needed for individuals to continue living at home. Dakota at Home also provides a service called Options Planning to educate and prepare people on how to meet future long-term care needs at home and in the community.

The Dakota at Home website offers additional information including frequently asked questions, how to make an online referral and access to various services and resources available across South Dakota through an online resource directory.

Providers who would like to be included in the online resource directory can find additional information here.
