SD DOH Office of Child and Family Services Survey

The South Dakota Department of Health (SD DOH) Office of Child and Family Services (OCFS) is asking interested parties to help gather information about the health and wellbeing needs of South Dakota Families, women, children, and youth with special healthcare needs. Input from this survey will assist with the development of South Dakota’s state plan for Maternal and Child Health (MCH) services for 2021-2025. The plan’​s main goal is to improve the health and well-being of these populations. Please consider taking and sharing the following survey link​.
