ROI for Chronic Disease Programs

The South Dakota Department of Health and its partners have developed a survey to determine how to assist health systems with implementing and sustaining evidence-based chronic disease programs for their patients. Health care professionals play a vital role in preventing and controlling chronic diseases through screening, monitoring, treatment and referrals to community health and education resources. Your feedback will help identify valuable outcomes and positive impact as well as develop resources that support your organization and communicate potential return on investment for evidence-based chronic disease programs. 

Any health care professionals involved in screening/testing/referring patients, including Physicians, Advanced Practice Providers, Nurses, Registered Dietitians, Social Workers, & Community Health Workers are encouraged to complete the survey.

The survey will close on Friday, October 30th.
