Healthcare professionals and systems are understanding the benefits of having palliative care available to their patients. Improving quality of life for their patients and their care partners comes through providing physical, emotional, spiritual and psychosocial support offered through palliative care’s interdisciplinary team. Systems find improved patient satisfaction and decreased cost of care by providing goal concordant care which can reduce ER and ICU visits.
Starting or expanding a palliative care program can come with many questions. The Center to Advance Palliative Care (CAPC) offers tool kits to help answer those questions. They include:
- Designing a Home-Based Palliative Care Program
- Hospice-Led Palliative Care
- Designing an Inpatient Palliative Care Program
- Designing an Office or Clinic Palliative Care Program
- Designing a Long Term Care Palliative Care Program
- Pediatric Palliative Care Delivery
- Concepts of Community Based Palliative Care Program Design 101
