The Centers for Medicare and Medicaid (CMS) has launched a website for the Rural Health Transformation (RHT) Program. The RHT Program was authorized by the One Big Beautiful Bill Act (Section 71401 of Public Law 119-21) and empowers states to strengthen rural communities across America by improving health care access, quality, and outcomes by transforming the health care delivery ecosystem. Through innovative system-wide change, the RHT Program invests in the rural health care delivery ecosystem for future generations.
States can apply for program funds starting mid-September with introductory webinars available in late September and states will be able to ask questions throughout October. States will have until early November 2025 to submit their one-time application for the RHT funds. Given the short turnaround time to complete the application plus state approval processes, it is expected submitting states will need to have their rural health transformation plan and application content completed by early to mid-October.
- Application instructions to apply for RHT Program funding will be included in the Notice of Funding Opportunity and released on grants.gov by mid-September
- CMS will engage with States in the open application period to answer questions
- Application submissions will close in early November
- Awards will be decided by December 31, 2025
