DHS Updates Process for Submitting DOH Facility Reporting Forms

The South Dakota Association of Healthcare Organizations (SDAHO) is sharing the following information from the South Dakota Department of Human Services (DHS) regarding an update to the reporting process for facilities using the Department of Health (DOH) Facility Reporting Form. We are providing this information as received to help ensure our members are aware of the change.

The South Dakota Department of Human Services (DHS) has implemented a change to the process for submitting reports using the DOH Facility Reporting Form.

Facilities will continue to submit reports to Department of Health.  In order to streamline the reporting process for facilities and reduce administrative burden, DOH reports that also need to be submitted to APS (Adult Protective Services) and the Ombudsman, can be sent to the DHS shared mailbox DHS.DOHReports@state.sd.us. This mailbox is monitored by both Adult Protective Services (APS) and the Long-Term Care Ombudsman Program, eliminating the need for additional routing of reports.

The Department of Health is in the process of updating the DOH Facility Reporting Form to reflect this new email address. Until the revised form is available, please use DHS.DOHReports@state.sd.us when submitting reports using the current DOH reporting form.
