LifeCircle, South Dakota’s Hospice and Palliative Care Network Targeted Education Committee invites clinicians, administrators, educators, researchers, quality improvement leaders, and interdisciplinary team members to submit abstracts for the Palliative and Hospice Care Track at the 2027Â Post-Acute Partners in Care Conference.
As the demand for high-quality, person-centered care continues to grow, acute, post-acute, and long-term care settings play a critical role in supporting individuals with serious illness, chronic conditions, frailty, and end-of-life care needs. This conference track will highlight innovative programs, clinical best practices, quality improvement initiatives, research findings, workforce development strategies, and interdisciplinary approaches that improve outcomes for residents, patients, families, and care teams.
