The South Dakota Association of Healthcare Organizations (SDAHO) is sharing a survey opportunity on behalf of the South Dakota Department of Health (SD DOH), Office of Lifespan Health, seeking feedback from health care providers regarding adolescent and reproductive health services in South Dakota.
Feedback will help inform future educational materials, outreach efforts, and program planning designed to support health care providers and improve access to high-quality care across the state.
The survey is part of the broader SD PLAN initiative, focused on provider engagement and developing resources informed by provider feedback.
The survey takes approximately 5–10 minutes to complete. Participation is voluntary, and individual responses will remain confidential.
