Grant money available for heart disease and stroke prevention programs

The South Dakota Department of Health and the Heart Disease and Stroke Prevention Program, together with the Centers for Disease Control and Prevention, announced that money is available to health care facilities to implement programs for preventing heart disease and stroke.

Applicants may apply for up to $5,000 for each of the following opportunities:

  • Implementation or expansion of PopHealth, a population health data analysis tool.
  • Implementation of policy and procedures around evidence-based quality measures.
  • Implementation or improvement of an aspect of team-based care.
  • Implementation or improvement of a self-measured blood pressure monitoring program.
