Overview:

This session will explore the critical role of immunization in public health and address the challenges posed by vaccine hesitancy. Presenter: Andrea Polkinghorn, BSN, RN​, President, Immunize South Dakota

Andrea Polkinghorn, BSN, RN is the board president for Immunize South Dakota, where she manages the business and affairs of the board. She oversees the coalition, achieving short-term and long -term goals, and presides over coalition meetings. In addition to her role with ISD, Andrea works as the immunization strategy leader for Sanford Health, where she develops and leads the implementation of immunization strategy for all vaccinating locations. She provides oversight and consultation to over 40 medical centers and more than 200 clinics to ensure standardization of immunization practices and appropriate implementation of immunization guidelines and provides ongoing immunization education for staff. Lastly, she is responsible for identifying, analyzing, implementing, and evaluating quality improvement measures to support a robust immunization program. ​

Andrea became passionate about vaccines in 2015 when she found herself questioning vaccines prior to her daughter’s birth. As a nurse, she heard some of her colleagues citing misinformation, which resulted in her questioning vaccine safety. Thankfully, her daughter’s pediatrician educated her about vaccines’ science and safety, and her daughter was vaccinated as recommended and on schedule. As Andrea learned more, she discovered the disparities in some immunization rates, and it ignited a passion within her to do what she can to educate ​and advocate for vaccination. Andrea graduated from South Dakota State University with a Bachelor of ​Science in nursing. She lives in Sioux Falls with her husband, Chris, her three daughters, and their dog, ​Diesel.

Between 15% and 20% of hospice patients are discharged while still alive. One-third of these patients die within six months, often without hospice readmission. Join preeminent researcher Stephanie Wladkowski to discuss the impact of this challenging reality and suggestions for improving both policy and practice. During 2023 and 2024, non-compliances with the Condition of Patient Rights have made this Condition the #2 most frequently cited Condition in the hospitals in South Dakota. Hospitals, critical access hospitals and rural emergency hospitals must comply with the Centers for Medicare & Medicaid Services (CMS) Conditions of Participation. According to CMS and accrediting organizations such as DNV and Joint Commission, Patient Rights and Responsibilities Condition is consistently in the top Condition Level Deficiencies cited. Within that condition, most non-compliances come from the regulations governing care in a safe environment, abuse, restraints and seclusion.

We will examine the effects of direct and indirect trauma. In helping roles, there is a close connection between yourselves and those you serve. This relationship may expose you to the distress and pain experienced by others, however, it may be strengthened through empathy and a desire to alleviate pain and suffering. We will examine the risks and recognition of trauma exposure, the ethical imperative of self-care, as well as coping strategies that contribute to health, well-being and resilience.

Jessica Broullire serves as the Education Specialist/Community Navigator for Children’s Home Child Advocacy Center. Jessica provides trauma informed education from parents to professionals from across the region. Jessica also provides education at local, state, and international events. Jessica also works with children and their caregivers who are seeking services due to allegations of child maltreatment. Jessica has dedicated her entire career to advocating for the wellbeing of children, firmly believing that keeping children safe is a community responsibility.

Overview:

In the long-term care setting it is likely that our residents will have one or more chronic disease diagnosis. We will explore some of the common chronic diseases we often see in the geriatric population and how to treat those disease states.

Presenter: Sandy Neyhart, Program Specialist, Services to the Blind and Visually Impaired

Sandy Neyhart is the Program Specialist with Service to the Blind and Visually Impaired (SBVI) in South Dakota where she is responsible for the Older Blind Independent Living Services program.  In addition to the Older Blind Independent Living Services program activities and requirements, she also supervises five Rehabilitation Teachers who provide low vision and skills of blindness training throughout SD. She contributes to the SBVI leadership team which includes professional development activities for staff, annual reports for Rehabilitation Services Administration, reviews program policies, quality assurance activities and exploring avenues of continuous improvement.

Mary Jackson, a survivor of human trafficking, will be sharing her personal journey to educate and raise awareness about human trafficking. She will specifically be discussing the indicators and red flags of possible trafficking that healthcare providers should be aware of. Mary will also be highlighting the vulnerabilities, safety concerns, and the range of care that trafficking survivors need. Her unique perspective as a survivor who has interacted with healthcare providers during her journey makes her insights especially valuable.

Presenter: Mary Jackson, Survivor Service and Program Coordinator, Call to Freedom

Mary Jackson is a speaker, educator, and advocator for those victimized by human trafficking, domestic violence, and sexual exploitation. She shares her own personal journey as a survivor of cartel trafficking. Her main goal is to educate, lending insight into what human trafficking looks like and sharing the vulnerability and criminality behind the crime. As coauthor of the book A Parent’s Guide to Understanding Human Trafficking, she has turned her suffering and pain into victory over this horrific crime, sharing her own struggles. Mary began serving at Call to Freedom in 2016 as a volunteer, joined the Board of Directors in 2020, and currently works full time supporting clients and educating. Through her work as the Survivor Service and Program Coordinator with Call to Freedom, she contributes vital expertise into survivor supportive services.

Mike McCormick is a life-long advocate for vulnerable people. He grew up in Sioux Falls, South Dakota and still thinks of it as home. He now lives in Rapid City with his wife, Chris, whom he has been married to for 36 years. He has two sons- Rheider and Connor. Mike graduated from Minot State University with a degree in Developmental Disabilities and Coaching. He also graduated from the Music Technical Institute in Minneapolis. He advocated in the field of developmental disabilities for twenty years before joining the Pennington County States Attorney’s Office in 2012. He worked for the States Attorney’s Office as the Domestic Violence Victims’ Advocate until he was promoted to the position of Director of Victim’s Assistance. Mike loves music, especially rock and roll. In his free time, he enjoys hunting, fishing, playing the drums, riding his motorcycle and going to concerts. His absolute favorite thing to do is to ride his Harley to concerts. In September of 2022 he joined the South Dakota Network against Family Violence and Sexual Assault.

She is an Associate Professor of Sandford School of Medicine and Instructs the Interdisciplinary Palliative Care Seminar hosted by the University of South Dakota’s Interprofessional Health Education Center.

Sarah Mollman is the Associate Dean for Research in the College of Nursing at South Dakota State University. Her past research includes exploring decision making in women with early-stage breast cancer and perceptions and knowledge of palliative care. She has also co-led a statewide network dedicated to improving awareness and knowledge of palliative care. Dr. Mollman is principal investigator on a study to adapt and test an early palliative care intervention for American Indian and rural patients with advanced cancer and their family caregivers. Sarah balances her professional life by spending time with her family (two and four-legged), gardening, and brewing kombucha.

With over 30 years in healthcare, Cheri works for the South Dakota Foundation for Medical Care. She is currently collaborating with the SD Department of Health on providing infection control guidance related to healthcare associated infections and performing facility Infection Control Risk Assessments.

She has worked with facilities, healthcare providers, community leaders and patients on multiple healthcare initiatives. She has received her certification in Infection Control and is anxious to work with facilities and frontline healthcare workers on infection prevention measures.

She is certified not only in infection control but has received certification in long term care infection control, case management and wound care. She played a pivotal role in the development of a national Home Health Infection Prevention Toolkit and was selected to serve as a technical reviewer for the national organization.

Cheri has worked as a Quality Risk Manager and Infection Control Nurse and also completed the AgriSafe Nurse Scholar class in 2019 and was selected to serve as South Dakota’s Total Farmer Health Coach, which promotes health and wellness to SD agriculture producers. She received the 2023 South Dakota Health Care Quality Professional of the Year and was recently selected to serve on a 3-year term for the national APIC Education Committee.

Significant challenges exist in providing safe and adequate care to the patient with co-occurring chronic pain and opioid dependence. Establishing trust with and empowering this patient to be an active participant in their health is a difficult but necessary step. During this session, we will focus on making an accurate diagnosis of their medical condition and developing an appropriate treatment plan in response. We will also review the significant differences in Morphine Milligram Equivalent (MME) dosages associated with acute pain, post-op pain, and chronic pain.

Objectives: 

• Identify the three health conditions that should be addressed in every clinic encounter for patients with chronic pain & opioid dependence (CPOD).
• Recognize the specific DSM-5 criteria for defining Opioid Dependence, Opioid Use Disorders, and Addiction.

Craig Uthe, MD is a board-certified family physician with the American Board of Family Medicine and a member of the American Society of Addiction Medicine. He is the Medical Director of Professional Well-Being at Sanford Health and the Chief Well-Being Officer for the University of South Dakota Sanford School of Medicine.

For over twenty-five years, Dr. Uthe has been a medical advisor to the SD Health Professionals Assistance Program, an organization that provides management services to health care professionals with potentially impairing illnesses & addictions.

LifeCircle Quarterly webinar: Palliative care access in rural communities can be a challenge, in part, due to workforce challenges. Community health workers can help expand access to other types of care, but what about palliative care? This session will summarize recent research on the perspectives of current palliative care providers and advocates on integrating community health workers into palliative care teams in South Dakota and will suggest opportunities for further discussion.

Presenter: Angela Kroeze Visser, Ph.D., MPH

Angela recently completed her dissertation research on community health workers in palliative care from the University of South Dakota. She also holds an MPH in Community Health Education from the University of Minnesota and has worked for more than 15 years in community public health roles. Her work with communities to identify and address health needs fuels her interest in helping implement programs in ways that are sustainable. In addition, Angela currently serves as the Director of the Kielstra Center for Research and Grants at Dordt University.

This program will review the concept of moral distress, and how it differs from other types of distress in healthcare. We will discuss strategies for recognizing and addressing conflict, both within ourselves and with others, and share mechanisms to build moral resilience and conflict competence.

Dr. Lindsay Semler is the Executive Director of Clinical Ethics and Department of Medicine Faculty at Brigham and Women’s Hospital, and a Lecturer at Harvard Medical School. She is also the Founder and President of Semler Ethics Consulting, LLC, which is available to organizations and clinicians who are looking to develop their ethics service or provide evidence-based workshops to address moral distress. Her background includes critical care and emergency nursing experience, as well as education, quality improvement, and leadership at UPMC and Penn Medicine. She was also the director of the system-wide Ethics program at INTEGRIS Health. Lindsay obtained her BSN and MSN from the University of Pittsburgh, Doctor of Nursing Practice degree with a focus on moral distress from Johns Hopkins, and the Advanced Training in Healthcare Ethics certificate from the University of Washington. She is certified as a Healthcare Ethics Consultant (HEC-C) and critical care nurse (CCRN). Her bioethical areas of interest focus on organizational and preventive healthcare ethics, addressing moral distress, and teaching future generations of healthcare providers.

She has her Masters of Social Science in Community Health.

Valorie is a member of Forvis Mazars Healthcare Performance Improvement team. She has extensive experience in Clinical Revenue Integrity Processes, Utilization Review and Case Management. Her strong clinical knowledge and understanding of government and commercial payment systems bring a practical perspective to clients. She has prior experience as a Provider, Clinician, Utilization Review Nurse, and Nurse Case Manager on the facility and payor perspectives.

She also supports the development and collaboration of strong Utilization Review and Case Management processes coupled with Clinical Documentation Integrity and Coding. She focuses on denials prevention and response, reimbursement trends related to the shift from inpatient to outpatient services, and clinical documentation supporting medical necessity. Valorie has assisted large children’s hospitals with Medicaid RAC audits.

Valorie is a member of the Healthcare Financial Management Association, Case Management Society of America, and American Association of Nurse Practitioners. She is a dual-certified nurse practitioner in Adult-Geriatric Medicine and Family Medicine, a Registered Nurse, and a Certified Case Manager. While working in surgical services as a circulator, as a scrub nurse, and in the PACU and pre-/post-op unit, she was Periop 101 certified through Association of PeriOperative Registered Nurses (AORN). She is a current member of AORN and American Society of PeriAnesthesia Nurses.

She is a graduate of Purdue University with a Bachelor of Science in Nursing and a Master of Science in Nursing: Adult-Gerontology Nurse Practitioner. She is also a graduate of University of Massachusetts Boston, with a Master of Science in Nursing: Family Medicine Nurse Practitioner.

This session aims to provide a comprehensive understanding of the behavioral health needs of children and adolescents in South Dakota, while highlighting the barriers to accessing care faced by communities throughout the state, particularly in rural and Tribal areas. Attendees will gain insights into the current challenges within the behavioral healthcare workforce and explore the Behavioral Health Education, Access, and Management for South Dakota (BEAM-SD) initiative, which seeks to enhance capacity and improve outcomes for pediatric behavioral health.

Dr. Eric Kurtz is the Executive Director of the Center for Disabilities at the University of South Dakota Sanford School of Medicine. The Center for Disabilities is South Dakota’s federally designated University Center for Excellence in Developmental Disabilities (UCEDD). Dr. Kurtz provides leadership and strategic direction for its core functions across clinical services, research, interprofessional education, training and technical assistance, information dissemination, as well as policy-related efforts. Dr. Kurtz is also the Principal Investigator for the South Dakota Leadership Education in Neurodevelopmental Disorders (LEND) program and Principal Investigator for the South Dakota Pediatric Mental Health Care Access Program. He also holds several additional appointments including Associate Professor of Pediatrics at the USD Sanford School of Medicine.

Nikki Eining is a private independent practice licensed social worker who serves as the Statewide Pediatric Mental Health Care Access Program Coordinator for The USD Center for Disabilities. Nikki brings a diverse set of experiences and passions spending over 18 years working in the mental health field. Her previous experience has focused on supporting youth and families in office, school, and community settings without outpatient behavioral health services and prevention programming. She has professional experience in clinical mental health, education, research design, non-profit, program development, and trauma-informed care practices.

Overview:

American Indian/Alaska Native (AI/AN) persons disproportionately suffer from diabetes and end-stage kidney disease (ESKD) compared to non-Hispanic whites (NHW). Over two-thirds of all ESKD in AI/AN persons is caused by diabetes (ESKD-D). Kidney transplant is the most desirable option to treating ESKD, but kidney transplant remains unattainable for many AI/AN persons, especially in South Dakota (SD). Furthermore, palliative and hospice care options for AI/AN with any serious illness in SD is largely inaccessible. For kidney transplant recipients, the likelihood of receiving palliative or hospice care is likely less because of the desire to prolong transplant function as long as possible. This presentation explores the identified disparities in hospice use among American Indian persons with ESKD-D to non-Hispanic Whites who have received a transplant, and those with no history of transplant.

1. Describe hospice use disparities for American Indian/Alaska Native persons with end-stage kidney disease.

Presenter: Brandon Varilek, Assistant Professor, University of Nebraska Medical Center

Dr. Brandon Varilek is an Assistant Professor at the University of Nebraska Medical Center College of Nursing. His program of research is focused on mitigating health disparities and addressing social determinants of health that affect South Dakota and Nebraska minoritized populations. Much of this work centers around persons with cancer or end-stage kidney disease. Dr. Varilek is also passionate about palliative care research and aims to expand his program of research to develop palliative care interventions for those on dialysis. Dr. Varilek is PI or Co-PI on two NIH grants. 1) NIDDK/CAIANDTR (P30) to explore survival & health disparities between American Indian/Alaska Native and non-Hispanic white populations with end-stage kidney disease; 2) AIM-AHEAD Consortium Development Award through NIH Office of Strategic Data Science to develop an explainable machine learning algorithm to generate mortality risk score based on social determinants of health for integration into clinical partner EMR systems.

Dunning is a recognized security and emergency management expert with certifications from the International Association for Healthcare Security and Safety, the Medical College of Georgia and the International Association for Emergency Managers. He holds an associate degree in law enforcement from The Community College of the Air Force, a bachelor’s degree in psychology from the University of Maryland and a master’s degree in emergency management from Jacksonville State University. Phillip Boaz is a Senior Consultant with Chartis Clinical Quality Solutions. He brings his clients more than 30 years of consulting and healthcare experience in the areas of critical care and trauma nursing, infection prevention, risk management, and regulatory compliance. With a background as a Centers for Medicare & Medicaid Services (CMS) surveyor in Texas, he leads full regulatory rehearsals (mock surveys) to prepare clients for upcoming regulatory visits. He also assists clients in crafting realistic and sustainable plans of correction in response to survey findings.

Prior to joining the firm, Phillip was System Director of Risk Management for a hospital system in East Texas, which included two hospitals, an ambulatory surgery center, and two off-site emergency departments. He was a CMS and state licensure surveyor for the Texas Department of State Health Services for nearly five years. There, he conducted over 130 comprehensive surveys and complaint investigations at regulated health facilities to determine compliance with state and federal regulations. Phillip was also the Infection Control Director for a full-service, 425-bed regional referral hospital for over six years and a Trauma Clinical Coordinator in a level 1, high-volume university teaching hospital for more than seven years.

Phillip is a Registered Nurse and received his Master of Science in nursing as an acute/critical care clinical nurse specialist from Vanderbilt University and his Bachelor of Science in biology and chemistry from Georgetown College. He is a member of the Association of Professionals in Infection Control and Epidemiology (APIC) and holds a Certification in Infection Prevention and Control (CIC) from APIC’s Certification Board of Infection Control and Epidemiology (CBIC).

Emily Johnson is a nurse consultant with South Dakota Medicaid, serving in the role as the PASRR program manager. In this role, she provides a broad range of technical assistance to providers on the PASRR process, Medicaid compliance, and quality assurance. Emily is also working towards redesigning the South Dakota PASRR program to ensure alignment with Federal regulations, enhance provider education, and streamline the overall process. She has over 13 years of nurse consultant service with State Government specializing in policy development, grant management, informatics, and quality assurance. Before joining South Dakota Medicaid, Emily served as Nurse Team Lead for the Department of Health where she oversaw nursing practice and services provided by the Community Health field offices in Central South Dakota. She also has had prior experience in PASRR, serving as the nurse consultant for the State Intellectual Disability Authority. Emily has a Bachelor of Science in Nursing from South Dakota State University as well as a South Dakota Governor Leadership Graduate Certificate.

Ariel Ghanayem currently serves as the South Dakota PASRR Program Manager with Maximus. Ariel has worked with Maximus since 2018 in a variety of roles, including working as a Quality Coordinator, Operations Manager, and Program Manager. In addition to South Dakota, she has also worked on the New York, Illinois, Wisconsin, and Virginia PASRR programs and currently manages the North Dakota PASRR and U21 programs in addition to South Dakota. Throughout her tenure at Maximus, she has assisted in multiple program implementations and has worked collaboratively with states to enact change, provide education, and streamline their PASRR programs.  Ariel has worked extensively with individuals’ mental health conditions, developmental disabilities, and substance abuse disorders. Her understanding of these populations contributes to excellent oversight of PASRR services for South Dakota. She brings over ten years of clinical expertise in conducting mental health assessments, developing treatment plans, and supporting individuals’ integration into community settings. The number of deficiencies in the physical environment condition of participation and the environment-of-care standards has continued to increase over the last several years. This session will review commonly cited life safety code deficiencies in hospitals. We will review commonly cited physical environment deficiencies and most frequently cited Life Safety Code deficiencies. CMS updates on these areas including expectations on ligature resistant environment, CMS categorical waivers, air pressure and humidity concerns in surgical suites.

Discharge planning has become more than just the movement of the patient out of the hospital.  It is a “process” that starts at the point of admission and follows beyond discharge and through the continuum of care. The Centers for Medicare and Medicaid Services have proposed more “teeth” to the process, with updates to the discharge planning section of the Conditions of Participation. You may spend most of your day focusing on utilization management and discharge planning, but care coordination is one of the key roles that hospital case managers and social workers perform. Other roles include utilization management and discharge planning. It is integral to managing cost and length of stay. Nevertheless, it is often overlooked or under-performed. This is sometimes due to workload issues and/or understaffing. Other times it is due to a lack of focus on this area of work.

Amy has been a national speaker on healthcare issues that impact older adults as well as the empowerment of their caregivers for more than two decades. She has served the primary care needs of older adults as a geriatric nurse practitioner for 24 years and was a tenured associate professor of nursing for over 15 years. She is recognized as a leading expert on dementia-related topics and has been appointed by three Kansas governors to serve on boards and task forces to represent the healthcare interests of older adults. Under Amy’s tenure as president of the Kansas Advanced Practice Nurses Association, Kansas became the 26th state to pass full practice authority for APRNs. In 2023 Amy was awarded the AANP Award for Excellence in the state of Kansas. She appreciates the opportunity to positively influence the lives of her patients and their caregivers.

An in-house acquired or declining pressure injury puts providers at risk for regulatory citations and litigation. The burden on the provider is to demonstrate that the development or decline of a pressure injury was unavoidable.  The interventions that are implemented within the first 24 hours are critical, as well as on-going interdisciplinary updates to the plan of care.  This session will utilize the QAPI approach for implementing or enhancing a pressure injury program to ensure your pressure injury program meets regulatory guidance for F686, as well as NPIAP and WOCN standards of practice to minimize regulatory and legal risk.

• Develop an interdisciplinary team approach and have effective team meetings that include prevention
• Demonstrate how to effectively utilize the wound care nurse to manage and oversee the program

Cross over symptoms of schizophrenia and dementia create challenges in assuring residents receive the appropriate diagnosis. A sharp rise in the number of reported schizophrenic cases in long term care centers across the nation has prompted CMS to conduct audits. She recently started working with SDAHO to support members providing Hospice and Palliative Care services and facilitates the LifeCircle SD group. When it comes to the prevention of pressure injuries, a 100% of the preventative interventions are performed by the caregivers. Few people have been with 10,000 dying Veterans, the founders of Opus Peace became the witness bearers for these Veterans as unbidden memories surfaced on their death beds.  They learned lessons about how to attain personal peace, and paradoxically, these lessons came from those who were trained for war!  These Veterans taught the founders of Opus Peace the following lessons:

• How to move from numbing their pain to mourning their pain.
• How to move from hiding guilt to learning how to forgive yourself and others.
• How to move from being ashamed of not being good enough to releasing your fear of who you are and who you are not.
• How to let yourself feel helplessness over things you had no control over rather than trying to control things you cannot control.

Accurate assessment of a pressure injury is important as it evaluates the effectiveness of the overall course of treatment and determines the type of topical support needed. Many nurses struggle with staging pressure injuries, especially upon admission. This course will provide the participant with the information needed to complete a comprehensive assessment of a pressure injury. This course will outline the NPIAP pressure injury staging system as well as wound measurement, wound base description, peri-wound, drainage, odor, tunneling, undermining and pain assessment.

Dr. Grant maintains a clinical practice as a palliative care nurse practitioner at the University of Maryland Medical Center. She is adjunct faculty at the University of Maryland and Johns Hopkins Schools of Nursing. In 2014, Dr. Grant was selected as a Robert Wood Johnson Health Policy Fellow working on Capitol Hill and at the Center for Medicare and Medicaid Innovation. Dr. Grant has served on the board of the Carolinas Center and the Hospice and Palliative Nurses Association and speaks nationally and internationally on palliative care, messaging, and health policy. Before becoming a nurse, Dr. Grant had a career in marketing at the Procter & Gamble Company and worked on the Max Factor and Cover Girl cosmetics brands there. She is the daughter of immigrants and refugees.

Many clinicians have difficulty differentiating lower extremity ulcers (arterial, venous, or neuropathy/diabetic) from pressure injuries.  If a lower extremity ulcer is mistaken as a pressure injury it can lead to improper treatment of these areas and skew your pressure injury rates. This session will help clinicians differentiate arterial insufficient ulcers, venous stasis ulcers and peripheral neuropathic/diabetic ulcers from pressure injuries and discuss how assessment and treatment strategies differ for each of these wound categories.

Karla has worked in community-based health for over 20 years, and is passionate about enabling older adults to remain active and independent. Karla provides nurse education as a Respecting Choices Facilitation Trainer, lead faculty for the Foundations of Faith Community Nursing Course, and Foot and Nail Care Certification training.  Currently, Karla is Co-Director for two Administration for Community Living Falls Prevention Grants at Sanford Medical Center, implementing A Matter of Balance, Stay Active & Independent for Life, Bingocize®, and CAPABLE.

Abby Gramlick-Mueller works at Sanford Health in Sioux Falls as a Nurse Scientist.  Abby has her Doctor of Nursing Practice from South Dakota State University and is certified as a Family Nurse Practitioner.  Abby works closely with the clinical teams in both the inpatient and ambulatory settings at Sanford to advance care delivery through innovation driven by evidenced based practice (EBP) and nursing research.

Abby’s work experience includes work as a registered nurse at Sanford in the Birthplace, the Endocrinology Clinic and in Adult Critical Care. She has worked as a nurse practitioner in the areas of family medicine, hospital medicine, chemical dependency, and occupational health. She also worked for a decade as the clinical director of a medical management company assisting with program and policy development.
