Phillip Boaz is a Senior Consultant with Chartis Clinical Quality Solutions. He brings his clients more than 30 years of consulting and healthcare experience in the areas of critical care and trauma nursing, infection prevention, risk management, and regulatory compliance. With a background as a Centers for Medicare & Medicaid Services (CMS) surveyor in Texas, he leads full regulatory rehearsals (mock surveys) to prepare clients for upcoming regulatory visits. He also assists clients in crafting realistic and sustainable plans of correction in response to survey findings.

Prior to joining the firm, Phillip was System Director of Risk Management for a hospital system in East Texas, which included two hospitals, an ambulatory surgery center, and two off-site emergency departments. He was a CMS and state licensure surveyor for the Texas Department of State Health Services for nearly five years. There, he conducted over 130 comprehensive surveys and complaint investigations at regulated health facilities to determine compliance with state and federal regulations. Phillip was also the Infection Control Director for a full-service, 425-bed regional referral hospital for over six years and a Trauma Clinical Coordinator in a level 1, high-volume university teaching hospital for more than seven years.

Phillip is a Registered Nurse and received his Master of Science in nursing as an acute/critical care clinical nurse specialist from Vanderbilt University and his Bachelor of Science in biology and chemistry from Georgetown College. Sandy Neyhart is the Program Specialist with Service to the Blind and Visually Impaired (SBVI) in South Dakota where she is responsible for the Older Blind Independent Living Services program.  In addition to the Older Blind Independent Living Services program activities and requirements, she also supervises five Rehabilitation Teachers who provide low vision and skills of blindness training throughout SD. Mary Jackson, a survivor of human trafficking, will be sharing her personal journey to educate and raise awareness about human trafficking. She will specifically be discussing the indicators and red flags of possible trafficking that healthcare providers should be aware of. Mary will also be highlighting the vulnerabilities, safety concerns, and the range of care that trafficking survivors need. Her unique perspective as a survivor who has interacted with healthcare providers during her journey makes her insights especially valuable.

Mary Jackson is a speaker, educator, and advocator for those victimized by human trafficking, domestic violence, and sexual exploitation. She shares her own personal journey as a survivor of cartel trafficking. Her main goal is to educate, lending insight into what human trafficking looks like and sharing the vulnerability and criminality behind the crime. As coauthor of the book A Parent’s Guide to Understanding Human Trafficking, she has turned her suffering and pain into victory over this horrific crime, sharing her own struggles. Mary began serving at Call to Freedom in 2016 as a volunteer, joined the Board of Directors in 2020, and currently works full time supporting clients and educating. Through her work as the Survivor Service and Program Coordinator with Call to Freedom, she contributes vital expertise into survivor supportive services.

Mike McCormick is a life-long advocate for vulnerable people. He grew up in Sioux Falls, South Dakota and still thinks of it as home. He now lives in Rapid City with his wife, Chris, whom he has been married to for 36 years. He has two sons- Rheider and Connor. Mike graduated from Minot State University with a degree in Developmental Disabilities and Coaching. He also graduated from the Music Technical Institute in Minneapolis. He advocated in the field of developmental disabilities for twenty years before joining the Pennington County States Attorney’s Office in 2012. He worked for the States Attorney’s Office as the Domestic Violence Victims’ Advocate until he was promoted to the position of Director of Victim’s Assistance. Mike loves music, especially rock and roll. In his free time, he enjoys hunting, fishing, playing the drums, riding his motorcycle and going to concerts. His absolute favorite thing to do is to ride his Harley to concerts. In September of 2022 he joined the South Dakota Network against Family Violence and Sexual Assault.

You may spend most of your day focusing on utilization management and discharge planning, but care coordination is one of the key roles that hospital case managers and social workers perform. Other roles include utilization management and discharge planning. It is integral to managing cost and length of stay. Nevertheless, it is often overlooked or under-performed. This is sometimes due to workload issues and/or understaffing. Other times it is due to a lack of focus on this area of work.

This program will review the elements that most greatly impact on cost and length of stay.  By focusing on care coordination as one of the key strategies for successful reductions of these metrics, you will be better positioned to perform your role as a hospital case manager. Care coordination, as one of the main roles of the RN case manager, is complex and includes many characteristics that encompass the role. Although these issues can be complex, there are tools and techniques to incorporate care coordination into your daily practice. While AUR reporting has been voluntary through the National Health Safety Network (NHSN) for several years, Centers for Medicare & Medicaid Services (CMS) programs like Promoting Interoperability are beginning to require AUR reporting. Joint Commission has been auditing components of antimicrobial stewardship (AMS) for several years and beginning in 2023, has begun to require AMS programs to track antimicrobial use using Days of Therapy (DOT) or AU reporting in surveys as a required element of performance. In 2024, AUR submission will be part of the Medicare Promoting Interoperability Program and required to satisfy the Public Health and Clinical Data Exchange objective. Hospitals (including Critical Access Hospitals) must use an ONC-certified vendor to submit to avoid downward adjustment in reimbursements for Medicare beneficiaries.

She is recognized as a leading expert on dementia-related topics and has been appointed by three Kansas governors to serve on boards and task forces to represent the healthcare interests of older adults. Under Amy’s tenure as president of the Kansas Advanced Practice Nurses Association, Kansas became the 26th state to pass full practice authority for APRNs. An in-house acquired or declining pressure injury puts providers at risk for regulatory citations and litigation. The burden on the provider is to demonstrate that the development or decline of a pressure injury was unavoidable.  The interventions that are implemented within the first 24 hours are critical, as well as on-going interdisciplinary updates to the plan of care.  This session will utilize the QAPI approach for implementing or enhancing a pressure injury program to ensure your pressure injury program meets regulatory guidance for F686, as well as NPIAP and WOCN standards of practice to minimize regulatory and legal risk.

A sharp rise in the number of reported schizophrenic cases in long term care centers across the nation has prompted CMS to conduct audits. Few people have been with 10,000 dying Veterans, the founders of Opus Peace became the witness bearers for these Veterans as unbidden memories surfaced on their death beds.  They learned lessons about how to attain personal peace, and paradoxically, these lessons came from those who were trained for war!  These Veterans taught the founders of Opus Peace the following lessons:

• How to move from numbing their pain to mourning their pain.
• How to move from hiding guilt to learning how to forgive yourself and others.
• How to move from being ashamed of not being good enough to releasing your fear of who you are and who you are not.
• How to let yourself feel helplessness over things you had no control over rather than trying to control things you cannot control.

Accurate assessment of a pressure injury is important as it evaluates the effectiveness of the overall course of treatment and determines the type of topical support needed. Many nurses struggle with staging pressure injuries, especially upon admission. This course will outline the NPIAP pressure injury staging system as well as wound measurement, wound base description, peri-wound, drainage, odor, tunneling, undermining and pain assessment.

Dr. Grant maintains a clinical practice as a palliative care nurse practitioner at the University of Maryland Medical Center. She is adjunct faculty at the University of Maryland and Johns Hopkins Schools of Nursing. In 2014, Dr. Grant was selected as a Robert Wood Johnson Health Policy Fellow working on Capitol Hill and at the Center for Medicare and Medicaid Innovation. Dr. Grant has served on the board of the Carolinas Center and the Hospice and Palliative Nurses Association and speaks nationally and internationally on palliative care, messaging, and health policy. Before becoming a nurse, Dr. Grant had a career in marketing at the Procter & Gamble Company and worked on the Max Factor and Cover Girl cosmetics brands there. She is the daughter of immigrants and refugees.

Many clinicians have difficulty differentiating lower extremity ulcers (arterial, venous, or neuropathy/diabetic) from pressure injuries.  If a lower extremity ulcer is mistaken as a pressure injury it can lead to improper treatment of these areas and skew your pressure injury rates. This session will help clinicians differentiate arterial insufficient ulcers, venous stasis ulcers and peripheral neuropathic/diabetic ulcers from pressure injuries and discuss how assessment and treatment strategies differ for each of these wound categories.

Karla has worked in community-based health for over 20 years, and is passionate about enabling older adults to remain active and independent. Karla provides nurse education as a Respecting Choices Facilitation Trainer, lead faculty for the Foundations of Faith Community Nursing Course, and Foot and Nail Care Certification training.  Currently, Karla is Co-Director for two Administration for Community Living Falls Prevention Grants at Sanford Medical Center, implementing A Matter of Balance, Stay Active & Independent for Life, Bingocize®, and CAPABLE.

Abby Gramlick-Mueller works at Sanford Health in Sioux Falls as a Nurse Scientist.  Abby has her Doctor of Nursing Practice from South Dakota State University and is certified as a Family Nurse Practitioner.  Abby works closely with the clinical teams in both the inpatient and ambulatory settings at Sanford to advance care delivery through innovation driven by evidenced based practice (EBP) and nursing research.

Abby’s work experience includes work as a registered nurse at Sanford in the Birthplace, the Endocrinology Clinic and in Adult Critical Care. She has worked as a nurse practitioner in the areas of family medicine, hospital medicine, chemical dependency, and occupational health. She also worked for a decade as the clinical director of a medical management company assisting with program and policy development.
