South Dakota MOST – A transportable Medical Order Honoring the Patient’s Wishes

​On March 27, 2019 Governor Noem signed into law Senate bill 118 to establish certain provisions regarding advance care planning and allow the execution of a medical order based on a standardized form which is transportable across all healthcare settings and in all facilities. The Medical Orders for Scope of Treatment (MOST) form outlines a plan of care respecting the patient’s wishes after being diagnosed with a terminal illness in the last year of life.  Patients and providers should become familiar with this form and include it as a tool in their discussion on end-of-life care wishes. The form provides medical orders regarding CPR-code or no code status, and level of medical intervention such as comfort measures, limited additional measures or full intervention.
