Application Process Now Open for the Guiding an Improved Dementia Experience (GUIDE) Model

The Center for Medicare & Medicaid Innovation (CMMI) has released guidelines and opened the application process for the Guiding an Improved Dementia Experience (GUIDE) model. This model will test a value-based reimbursement system for participating providers and will include a dementia care navigator for the patients of those providers. The Request for Applications (RFA) (PDF) provides further information on eligibility requirements and additional details.

Qualifying clinics can submit their applications via the CMMI web portal by the deadline of Tuesday, January 30, 2024, at 11:59 PM EST.

The Alzheimer’s Association can assist in assessing different approaches to meet the requirements. If there are areas in the RFA that don’t apply to your system, their health systems team can help your team talk through the requirements, challenges, and workflow.
