Over the last few weeks, several tools, both for healthcare professionals and patients, have been in the news.  Here is a summary of the tools and links where you can get more information:

  • Professionals
    1. Tools for tomorrow: a scoping review of patient-facing tools for advance care planning a journal article that reviews “99 unique patient-facing tools… It demonstrates a broad array of innovative ACP tools that facilitate personalized and effective ACP…finding contribute to an enhanced understanding of their utilization & potential impacts, offering valuable insight for future tool development and policy making in ACP.”
    2. American Society of Clinical Oncology (ASCO) new recommendations for palliative care at the time of diagnosis.
    3. IHI Age friendly mobility assessment tools to be used in a variety of care settings
    4. GeriatricPain.org is a one-stop website from the University of Iowa Csomay Center for Gerontological Excellence with free best-practice tools and resources that support recommendations for quality pain assessment and management in older adults.
    5. National Quality Forum (NQF) practice guidelines for Hospice and Palliative Care state “a social assessment plan should address sexuality/intimacy, an area frequently overlooked in social planning.”  CAPC provides an overview and questions to help guide the conversation
