We will break down and simplify all of the changes occurring this year and proposed for next year in this session- and get to the facts of “what does this mean to me and my agency?!”

- The Home health proposed rule for 2025– from payment changes to quality proposals
- The HHVBP updates and first annual report-
- The NEW shift to GG’s in the HHVBP Discharge Function Measure
- The NEW use of observation stays in the Potentially Preventable Hospitalization- to be featured in 2025 HHVBP and in your OCTOBER 2024 Star rating!
- OASIS E-1 begins January 1, 2025– and the good news is that the changes are all POSITIVE!

Presenter: Annette Lee, RN, MS, COS-C, HCS-D, Provider Insights, Inc.
Annette is a registered nurse, with a Master’s in Health Care Administration, practicing since 1990. For over a decade she worked with the CMS Home Health and Hospice intermediary, Cahaba GBA and CGS, where she provided review of, and education on home health and hospice documentation. She worked as a specialist in the Medical Review, Appeals, and Provider Education departments, and assisted with cross-MAC quality assurance and training.
Annette then founded Provider Insights, Inc. providing consultative support and provider education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers across the nation with Survey Preparation, ADRs and appeals. Today she continues to marry together her experience in the “real world” and the inside knowledge of Medicare to ensure providers can meet the requirements of CMS, and ensure compliant, efficient operations.

Caregivers are the backbone of the US health care system, particularly for people living with serious illness such as Alzheimer’s Disease and other dementias. While palliative care goes a long way in supporting these patients, there continue to be more opportunities to optimize how we support caregivers. These range from better integrating caregivers into goals of care conversations and facilitating practical training to prepare for the physical and mental aspects of caregiving in the home, to connecting them with meaningful resources in their communities.
In this webinar, CAPC’s Andy Esch, MD, MBA, is joined by leaders from the Alzheimer’s Association and Hospice of the Valley’s Dementia Care and Education Campus to discuss this and more. Together they will share strategies for how to better support caregivers, including addressing some of the structural limitations that can hinder this work. Participants will leave with tools to implement targeted changes that can greatly improve the outcomes of caregivers and patients alike.
