The Hallmarks of a Successful Post-Acute Care Network

What are the hallmarks of a successful post-acute care network? Join us on October 7th at  in Sioux Falls to find out! This physician education event is intended to bring clinical and administrative leaders together to network and discuss the importance of collaboration, interoperability, care transitions and end of life care. Physicians will also get an overview of their role in the new long term care Patient-Driven Payment Model (PDPM) and South Dakota MOST (Medical Order for Scope of Treatment).
