Overview:

During this session, participants will be provided an overview of the SD MOST regulations by Michele Snyders, SDAHO’s Hospice and Palliative Care Program Manager. Ms. Snyders will identify healthcare providers who can legally complete a MOST, along with patients who qualify for a MOST. She will also review how to complete, store and activate a MOST.

  4. Learner will be able to express an understanding of how to complete, store and activate a MOST

Presenter: Michele Snyders, Hospice and Palliative Care Program Manager, SDAHO

Michele is a clinical social worker with 26 years of healthcare experience. She has worked in both inpatient and outpatient settings, always working with patients living with severe, chronic diseases and their families. In 2007 she began working in Palliative Care. She holds a Post Masters Certification in Hospice and Palliative Care from California State University Shiley Haynes Institute for Palliative Care and is Advanced Palliative and Hospice Social Work-Certified. She recently started working with SDAHO to support members providing Hospice and Palliative Care services and facilitates the LifeCircle SD group. LifeCircle is an inter-professional, inter-agency group of professionals who are committed to improving the lives of those with serious illness through education and advocacy.
