The Great Plains Quality Information Network Care Coordination Team is offering a webinar series to help organizations build capacity for better, safer care and healthier people and communities.
The first session, Care Coordination Overview, is planned for Tuesday, Oct. 25 from noon to 1 p.m. CDT, 11 a.m. to noon MDT.
The learning events aim to assist communities in reducing rehospitalizations, improving medication safety and improving chronic disease management.
The webinars will help create a platform for peer-to-peer learning, facilitate community collaboration across the GPQIN region (Kansas, Nebraska, North Dakota, and South Dakota), identify best practices and provide resources, tools and technical assistance to communities.
