In 2016, following the dissolution of the South Dakota Culture Change Coalition, remaining coalition funds were spent to purchase several toolkits from the Pioneer Network titled “Engaging Staff in Individualizing Care: An Implementation Handbook.” SDAHO has two of these toolkits available for use by its members.
The toolkit was developed through the Pioneer Network’s National Learning Collaborative, of which the SD Culture Change Coalition was actively involved. The toolkit includes a binder of information and a companion DVD on the following topics:
- Consistent Assignments
- Shift Huddles
- Quality Improvement Closest to the Residents
- Involving CNA’s in Care Planning
