An Advance Care Planning (ACP) fact sheet, dated June 2018, is now available from the Centers for Medicare & Medicaid Services (CMS) Medicare Learning Network (MLN). The fact sheet defines the service, explains patient eligibility and describes the Current Procedural Terminology codes 99497 and 99498.
There are no place-of-service limitations on ACP services. ACP services can be provided in facility and non-facility settings and services are not limited to a particular physician specialty. CMS does not require a specific diagnosis to bill the ACP codes. Critical access hospitals (CAHs) may bill for ACP using type of bill 85X with revenue codes 96X, 97X, and 98X.
