My Health Checklist

The Conversation Project, known for their conversation guides for having goals of care conversations, announces a new tool for you to consider for your patients. According to The Conversation Projects website, ā€My Health Checklist is designed to help older adults get the most out of their medical appointments. It helps them think through all aspects of their health- whatā€™s going well, what could be better, and your questions or concernsā€. Using the Age-Friendly 4 Mā€™s, what Matters, Medication, your Mind and your Mobility, it helps your patients be better prepared for their appointments so you can focus on the things that matter most to them.
