Visit SDAHO for recent Nursing Home Survey Results

Are you interested in seeing what deficiencies are being identified in recently posted nursing home surveys? The Department of Health posts the full survey inspection reports (form CMS-2567) on their website, and The South Dakota Association of Healthcare Organizations (SDAHO) creates an easy-to-read summary of the hundreds of reports posted.

SDAHO members can visit the nursing home page, and click on the link at the top of the page.

What you will find:

  • The first page of the PDF report shows the most common tags that the Department of Health has cited in 2026.
  • The following pages include a brief synopsis of findings for every deficiency in the state this year.

These reports will help your organizations see what other South Dakota nursing homes are being cited for as you enter your survey eligibility window, and can also be used as a proactive quality improvement tool to strengthen your internal processes anytime. The report is updated monthly.
