Overview:
Dakota at Home is South Dakota’s Aging and Disability Resource Center and acts as the “front door” to services and supports available through the Department of Human Services. This session will provide an overview of Dakota at Home, ways in which referrals can be made, how referrals are processed, and a brief overview of the services available through DHS/LTSS.

Presenter: Jennifer Gant, Program Manager – Dakota at Home, SD Dept of Human Services LTSS
Jennifer is the Program Manager for Dakota at Home. She has worked for the organization for 13 years and was active in the initial development and implementation of Dakota at Home, South Dakota’s Aging and Disability Resource Center. Jennifer has a Master of Science Degree in Criminal Justice from the University of Wisconsin, specializing in Victim and Offender Services. Jennifer also holds the credential of Certified Community Resource Specialist – Aging/Disabilities (CRS-A/D) through AIRS (Alliance of Information and Referral Systems).
