Overview:

Advance Care Planning is a process by which healthcare providers can help their patients express what matters most to them should they become seriously ill or injured in order for them to receive goal concordant care. This presentation will discuss the what, why, when and how of the Advance Care Planning process in order for you to incorporate it into your daily work with your patients.

Objectives:
  • Identify what Advance Care Planning is and what it is not
  • Describe why Advance Care Planning is an important part of patient care
  • Name five different events when Advance Care Planning should be done
  • Define the C-A-R/R-E elements of how to do Advance Care Planning

Presenter: Michele Snyders, MSW, CSW-PIP, APHSW-C, Hospice and Palliative Care Program Manager, SDAHO

Michele is a clinical social worker with 26 years of healthcare experience. She has worked in both inpatient and outpatient settings, always working with patients living with severe, chronic diseases and their families. In 2007 she began working in Palliative Care. She holds a Post Masters Certification in Hospice and Palliative Care from California State University Shiley Haynes Institute for Palliative Care and is Advanced Palliative and Hospice Social Work-Certified. She recently started working with SDAHO to support members providing Hospice and Palliative Care services and facilitates the LifeCircle SD group. LifeCircle is an inter-professional, inter-agency group of professionals who are committed to improving the lives of those with serious illness through education and advocacy.
