Purpose: Despite the strong evidence of benefit to patients and families, and increased demand for outpatient palliative care, many organizations are struggling to build sustainable models of outpatient palliative care. This webinar will provide information about palliative care clinics in different settings, current models, and review examples in diverse settings.
Objectives:
- Identify the key components of outpatient palliative and supportive care clinics: structure, staffing and services provided
- Discuss disease-specific palliative care, including referral process, palliative care provider roles, multidisciplinary team approach, and importance of care coordination
- Explain the role of palliative care outpatient clinics as extensions of an inpatient palliative care services
