Sepsis is the leading cause of death in U.S. hospitals. Sepsis is also the leading cause of readmissions to hospitals, with as many as 19% of people originally hospitalized with sepsis, re-hospitalized within 30 days.

This learning collaborative has been developed using the newly released Centers for Disease Control and Prevention (CDC) Hospital Sepsis Program Core Elements.
