The South Dakota Department of Health Office of Chronic Disease Prevention and Health Promotion, in collaboration with chronic disease partners and stakeholders, is conducting an environmental scan guided by the South Dakota Chronic Disease State Plan.

The purpose of the scan is to understand how community, school, worksite, tribal and health care sectors use technology to deliver public health messaging that enhances coordinated chronic disease prevention and health in South Dakota. This short Environmental Scan Survey is focused on identifying gaps as well as strengths in how technology is used to deliver public health messaging in South Dakota.

Please allow about 10 minutes to complete the survey, as responses will provide valuable information to guide public health in South Dakota.
