Home Health & Hospice Providers FISS DDE User ID Annual Recertification

Each year, Medicare providers are required to recertify their Fiscal Intermediary Standard System (FISS) Direct Data Entry (DDE) user access.

September 30, 2019 is the last day to complete the Annual DDE PPTN Recertification form. Failure to recertify will result in the termination of FISS DDE/PPTN services.

What You Need to Do
  • Complete the Annual DDE PPTN Recertification Form as soon as possible. Verify all User IDs, indicate if the User ID is active or inactive, and include an authorized signature, contact email, and phone number.
    • Note: This form is not used to add new users, delete users, or to update current users. To add, delete, or update User ID information, you must complete an Online Inquiry Services form. This form is not used for myCGS Recertification.
  • FAX the Annual DDE PPTN Recertification Form to CGS at: 1.615.664.5947

Hospice Providers: Please be aware that failure to recertify your FISS DDE access will result in the termination of your DDE User ID. This may cause untimely filing of your hospice Notices of Election (NOEs) and an exception may not be granted.
