2024-2025 Serious Illness Care Learning Collaborative

Ariadne Labs, developer of the Serious Illness Conversation Guide, announces the 15 month Learning collaborative that will start in the spring of 2024. According to Ariadne Labs “the Serious Illness Care Program (SICP) enables health systems to systematically engage patients in more, better and earlier conversations about their goals, values, and priorities so that all persons feel known and cared for on their own terms.”

In looking at the research around the SICP, the program results in “more, earlier, and better serious illness conversations and reduction in anxiety and depression for patients.”  It also has shown reductions in total medical expenses.  

The learning collaborative will be an online learning platform that will provide modules, tools to implement the program and downloadable resources. Systems will have 100 seats available to train interprofessional clinicians on the electronic Serious Illness Conversation Guide including:

  • Learning session on best practices and implementation guidance lead by the network’s experts in the field
  • Peer driven sessions to share implementation experiences, challenges, and solutions.
  • Coaching and support from implementation faculty and clinicians
  • Targeted areas for activity such as interprofessional, engaging patients and caregivers, measuring results and EHR integration.
