With COVID-19 no one can eliminate the uncertainty about whether they will get sick, or how sick they will become. But there is a way to limit the uncertainty, through proactively asking about the type of care they want, should they become severely ill. Providing this information in advance limits the uncertainty for their loved ones and for the clinical teams who can confidently create a plan that aligns with what matters most to the individual.

Join a panel of experts who will discuss resources available to help facilitate person-centered care conversations with patients and families related to the COVID-19 pandemic. Hear from colleagues on some of the unique challenges faced when having person-centered care conversations and review the legal considerations with the various advance care planning forms and documents.
