Mobilizing Patient Social Supports to Improve Health Outcomes

Since 1997, the CaringBridge communications platform has preserved critical personal connections by mobilizing a patient’s community to surround a patient and family caregiver with social, emotional, and instrumental support. Without personal social connections in a hospital or during recovery, a patient can easily become isolated and lonely, which can lead to serious health complications.

Tia Newcomer, CEO, CaringBridge, will share how the platform works and how it enhances patient health outcomes by mobilizing social connections. Also joining Newcomer from CaringBridge’s hospital partner, UMass Memorial Medical Center, is Alicia Wierenga, Senior Director of Patient- Family-Centered Care; and Billie Adler, Patient Experience Director of Patient- Family-Centered Care.
