Webinar: Discharge Planning: Compliance with the New CMS Hospital & CAH CoPs

Overview: During this program, Sue Dill Calloway will discuss the Improving Medicare Post-Acute Care (IMPACT) Act and how it impacts hospital discharge planning. She will review the new regulations (published on Sept. 26) on timely patient access to medical records, the discharge planning process, discharge instructions and planning requirements. She will also review transfers to other facilities; assessment of 30-day readmissions; caregiver rights and recommendations;  reduction of factors that lead to preventable readmissions; and timely discharge planning. Calloway will identify what was not adopted by Centers for Medicare and Medicaid (CMS). In many sections that were not adopted, CMS made recommendations to hospitals. Additionally, she will briefly discuss the final surveyor worksheet for assessing compliance with the CMS hospital Conditions of Participation (CoPs) for discharge planning.
